We focus on a very specific group of individuals that otherwise doesn’t have access to primary care options designed to meet their complex medical and long-term support needs. They are often forced to deal with short office visits, long wait times for urgent care and transportation complications. When medical complications develop, they tend to seek care at an emergency department, which often can lead to hospitalization and, eventually, long-term care placement.
