Patricia ‘Patsy’ Sullivan, MS, RN, NP-C
Nurse Practitioner
As a Nurse Practitioner, Patsy serves as the primary care provider for a panel of about 60-75 patients and as the locus of care coordination for her panel on our interdisciplinary team. She visits patients regularly in their homes conducting new intake, follow up, and annual wellness appointments and responds to requests for urgent care visits when new issues arise. Patricia prescribes medications, assesses care needs, writes care plans, orders DME and supplies and makes referrals to specialists when needed.
Patsy has over four decades of experience working as a Registered Nurse, and then a Nurse Practitioner in hospitals and health centers in Greater Boston, including Beth Israel Hospital, Neponset Health Center, and Community Medical Alliance working in a variety of outside-the-clinic settings. Most recently, she served as a Family Nurse Practitioner at Commonwealth Care Alliance. Throughout her career, Patricia’s passion has been caring for patients that are very medically complex, marginalized and whose access to quality health services is limited. Patricia holds certifications as a Nurse Practitioner and Family Nurse Practitioner from the American Academy of Nurse Practitioners.
